$ 40.00/person for CT ACHCA Members and guests registering with a CT ACHCA Member
$ 60.00/person for not-yet-a-CT ACHCA Member; $ 40.00/person for additional people (same company)
Mail your registration with check – OR – submit your Registration by fax and pay by check at the door. Registration is a commitment to pay. Survey Guarantee in effect; substitutions allowed.
